These POLICIES concern all Summer Camp by Bellevue VBS activities provided by Summer Camp by Bellevue VBS, including but not limited to, all Summer Camp by Bellevue VBS actives, by video, group chat, prior recording, or any other electronic means, provided by Summer Camp by Bellevue VBS and hosted on any third party platform (such as Zoom). As well as Summer Camp by Bellevue VBS occurring in person.

This Agreement is made and entered into by and between you and any person helping you visit, access, register with, and/or use the Summer Camp by Bellevue VBS website (collectively, “you” or “your” or "I"), on the one side, and Summer Camp by Bellevue VBS and Calvary Chapel Eastside, on the other side. All matters described with the organizational name of Summer Camp by Bellevue VBS, also applies to Calvary Chapel Eastside, both directly and indirectly. Please read this Agreement carefully before accessing or registering for Summer Camp by Bellevue VBS and Calvary Chapel Eastside, because it constitutes a legally binding agreement between you and the organization.

When you access the Summer Camp by Bellevue VBS website you are automatically accepting and agreeing to the most-recent version of this Agreement and your continuing visit, access, or use of the Summer Camp by Bellevue VBS website reaffirms your acceptance and agreement in each instance. Furthermore, by visiting, accessing, or using the Summer Camp by Bellevue VBS website you are automatically accepting and agreeing to the Terms of Use and Waiver for Summer Camp by Bellevue VBS website and services, which can be found at www.bellevuevbs.com. If you do not accept and agree to this Agreement in its entirety, then you are strictly prohibited from visiting, accessing, and/or using the Summer Camp by Bellevue VBS website and services.

Summer Camp by Bellevue VBS may supplement, amend, or otherwise modify this Agreement at any time. Such modifications will be posted on this or a similar page of the Summer Camp by Bellevue VBS website, as applicable, and shall be deemed effective as of modification date. The most recent updated version of the policies will be the ones held to your accountabilities upon arrival of events and upon registration for events. It is your responsibility to carefully review this Agreement each time you visit, access, or provide information to us at Summer Camp by Bellevue VBS and Calvary Chapel Eastside.

TERMS, AGREEMENTS, DISCLAIMERS, & WAIVERS

AFFILIATION
Summer Camp by Bellevue VBS is affiliated with Calvary Chapel Eastside for its Non-profit status. Please see www.bellevuevbs.com/contact-us/about-us/affiliation for more details and inquiries.

MASKS
Masks are considered OPTIONAL through out the camp. We, at Summer Camp by Bellevue VBS, believe you, as the parent/guardian are able to provide sound judgment on the health and safety of your children. We will however provide sanitizing stations throughout the building for you, and your children's convenience.

REQUIRED
Each child Registered is REQUIRED to have their own electronic device and must know how to use it, to Participate in our Virtual Camp!

OTHER COUNTRY & NATION WIDE
Summer Camp by Bellevue VBS and Calvary Chapel Eastside is not responsible for connectivity issues regarding home connections. all other disclaimers, terms and agreements apply regardless of country.

ATTENDANCE
Attendance of registered guests will be taken, & non- registered attendees will be removed from the call for Security reasons. DO NOT forward the links to non-registered parties.

PHOTO RELEASE
I hereby grant the Summer Camp by Bellevue VBS permission to use my likeness, or my children’s likeness, in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Summer Camp by Bellevue VBS and will not be returned. I hereby irrevocably authorize the Summer Camp by Bellevue VBS to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Summer Camp by Bellevue VBS from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

FEES
I understand that there will be No Refunds given after registering my Child/Children with Summer Camp by Bellevue VBS programs and that I am responsible for paying for the full specified fee for the Summer Camp by Bellevue VBS programs.

LIABILITIES
By agreeing to these Terms & Agreements in this release, I agree to indemnify and hold harmless Summer Camp by Bellevue VBS, Calvary Chapel Eastside, the Rented facility, and Staff from all parties, from injury, liability, expense or loss, as a result of my child (listed in this form) participating in Summer Camp by Bellevue VBS.

TEACHING
Summer Camp by Bellevue VBS stands firmly in the Truths of the Bible and proselytizes about it openly. Summer Camp by Bellevue VBS will teach truths according to Jubail-Christian Values. Therefore, Summer Camp by Bellevue VBS will not support, teach, or stand with anything that is not supported by the Bible. Anyone attending any of the Summer Camp by Bellevue VBS's and Calvary Chapel Eastside's events are agreeing to be taught by these values by default by showing up to attend the events and checking the agreement checkbox while registering.

YOUR DEVICES
You are responsible for obtaining and updating the Device, software, operating system, carrier, and network access necessary to properly access and use Summer Camp by Bellevue VBS services and activities. Summer Camp by Bellevue VBS does not guarantee that the services/activities, or any portions thereof will function on or in connection with any particular Device, software, operating system, carrier, or network. If you visit, access, or use the Summer Camp by Bellevue VBS website or services through a particular Device, then you hereby acknowledge and agree that information about your use of the Summer Camp by Bellevue VBS website and services through that Device or its carrier or network (such as, by way of example only, the identity of your Device or its carrier or network) may be communicated to Summer Camp by Bellevue VBS and/or certain third parties. The voice, message, and data fees, rates, charges, and taxes of your Device’s carrier or network, or another third party, may apply to your visit, access, and/or use of the Summer Camp by Bellevue VBS website or services. Summer Camp by Bellevue VBS is not responsible for, and you further accept full responsibility for, all Device, carrier, and network fees, rates, charges, and taxes which may apply, if any.

LICENSE OF USER MATERIALS
All names, photographs, information, communications, and any other content that you submit to or publish on an online social media account (e.g., Facebook, Instagram) that you own and link or otherwise associate with Summer Camp by Bellevue VBS. You hereby grant Summer Camp by Bellevue VBS an irrevocable, non-exclusive, royalty-free, fully-paid, transferable, sub-licensable, perpetual, and universe-wide license for Company to host, store, reproduce, transmit, distribute, sell, resell, license, sublicense, market, modify, adapt, create derivative works, communicate, publish, syndicate, publicly perform, publicly display, archive, and otherwise use and exploit all or any part of such User Materials and any elements and derivatives thereof in any manner, medium, or form, whether now known or hereinafter devised, as Company sees fit in its sole discretion.4

ORGANIZATIONAL RECORDINGS
By participating in Summer Camp by Bellevue VBS you will see and hear and can elect to be heard and seen by the other Users participating in Summer Camp by Bellevue VBS. By electing to display your image and or audio during participating in Summer Camp by Bellevue VBS, you consent to the video and/or oral recording of You and anyone around you throughout the entire duration of the camp and calls. Further, You hereby consent to the photographing, filming, taping, recording, interviewing, and/or referencing of you by Summer Camp by Bellevue VBS (itself and/or by or through its agents, employees, designees, and/or contractors) during your participation in all or any part of Summer Camp by Bellevue VBS. In addition, you hereby acknowledge and agree that Summer Camp by Bellevue VBS is, and that you have always known Summer Camp by Bellevue VBS to be, the exclusive owner of all past, present, and future rights, titles, and interests in and to all of your contributions to the resulting products of those photographing, filming, taping, recording, interviewing, and/or referencing activities (including, without limitation, all copyrights, results, and proceeds, as well as all allied, ancillary, and subsidiary rights in the foregoing, and all moral rights of the author or droit moral in the foregoing) (collectively, the “Recorded Works”), if any, as “works made for hire;” but if any of the foregoing is not a “work made for hire,” then you forever irrevocably transfer the same to Summer Camp by Bellevue VBS.

WARNINGS; DISCLAIMERS
Please note that the User Materials might become the subject of public disclosure. Thus, Summer Camp by Bellevue VBS and Calvary Chapel Eastside is not responsible for, and expressly disclaims any liability arising from or in connection with, its use of any User Materials in accordance with the terms and conditions of this Agreement.

REFUSAL
Summer Camp by Bellevue VBS has the right, but not the obligation, to refuse the Summer Camp by Bellevue VBS activities, and the Services in the event you are found to be disruptive, disorderly, loud, harassing, abusive, or a safety risk to any of Summer Camp by Bellevue VBS’s Volunteers, representatives, or participants, or if you are found to be negligently or recklessly using and/or abusing any equipment, property, or Services of Summer Camp by Bellevue VBS. Further, Summer Camp by Bellevue VBS may contact your payment method issuer, law enforcement, or others and share information relating to your payments if Summer Camp by Bellevue VBS believes doing so will prevent a violation of the law or financial loss.

REMOVAL
Summer Camp by Bellevue VBS has the right to remove you from virtual meetings if you are found to be disruptive, disorderly, loud, harassing, abusive, or a safety risk to any of Summer Camp by Bellevue VBS’s Volunteers, representatives, or participants. Summer Camp by Bellevue VBS has the right to remove you from virtual meetings if you do not comply with decisions and requests made by Summer Camp by Bellevue VBS Staff. Summer Camp by Bellevue VBS has the right to remove you from virtual meetings if you or your children keep playing with virtual backgrounds and animations. Summer Camp by Bellevue VBS has the right to remove you from virtual meetings if your video is turned off.

EXPRESS CONSENT
By submitting any information through the Summer Camp by Bellevue VBS contact forms and pages, you agree that Company may send you e-mails and other electronic messages directly and/or through the Summer Camp by Bellevue VBS website (such as, for example, regular or promotional e-mails, and push notifications, provided that you have approved the regular or mobile version of the Workouts to send push notifications). Further, you hereby expressly consent to Summer Camp by Bellevue VBS sending or otherwise communicating with you via any electronic means that Summer Camp by Bellevue VBS deems appropriate in its sole discretion (including, without limitation, telephone, e-mail, electronic message through Summer Camp by Bellevue VBS's, social media, or other electronic media formats), whether by personal communication (i.e., human-to-human), by e-mail, by automatic telephone dialing system, by an artificial or prerecorded voice, or by text message (e.g., short message service a/k/a SMS, and multimedia messaging service a/k/a MMS); and that, by giving such consent, you agree that no such communication shall violate the Telephone Consumer Protection Act, the CAN-SPAM Act, or any other applicable laws, rules, or regulations. Voice, message, and data fees, rates, charges, and taxes may apply to you, and you are responsible for payment of the same. You are required to grant the foregoing consent as a condition for purchase of Summer Camp by Bellevue VBS’s services.

DUTY TO SELF-MONITOR
Participants and volunteers agree to self-monitor for signs and symptoms of COVID-19 (symptoms typically include fever, cough, and shortness of breath) and, contact ACS at coronavirus@cancer.org if he/she experiences symptoms of COVID-19 within 14 days after participating or volunteering with ACS.

DONATIONS
All donations to Summer Camp by Bellevue VBS & Calvary Chapel Eastside are non-refundable.

LIABILITY WAIVER AND RELEASE OF CLAIMS:
You acknowledge that you derive personal satisfaction and a benefit by virtue of my participation and/or voluntarism with Summer Camp by Bellevue VBS, and you and your children willingly engage in h Summer Camp by Bellevue VBS events and/or other fundraising activities (the “Activity”).

RELEASE AND WAIVER.
YOU HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE ANY AND ALL LIABILITY, CLAIMS, AND DEMANDS OF WHATEVER KIND OR NATURE AGAINST Summer Camp by Bellevue VBS & Calvary Chapel Eastside AND ITS AFFILIATED PARTNERS AND SPONSORS, INCLUDING IN EACH CASE, WITHOUT LIMITATION, THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, AND AGENTS (THE “RELEASED PARTIES”), EITHER IN LAW OR IN EQUITY, TO THE FULLEST EXTENT PERMISSIBLE BY LAW, INCLUDING BUT NOT LIMITED TO DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE, FAULT OR CONDUCT OF ANY KIND ON THE PART OF THE RELEASED PARTIES, INCLUDING BUT NOT LIMITED TO DEATH, BODILY INJURY, ILLNESS, ECONOMIC LOSS OR OUT OF POCKET EXPENSES, OR LOSS OR DAMAGE TO PROPERTY, WHICH YOU, & YOUR HEIRS, ASSIGNEES, NEXT OF KIN AND/OR LEGALLY APPOINTED OR DESIGNATED REPRESENTATIVES, MAY HAVE OR WHICH MAY HEREINAFTER ACCRUE ON YOUR BEHALF, WHICH ARISE OR MAY HEREAFTER ARISE FROM YOUR PARTICIPATION WITH THE ACTIVITY.

ASSUMPTION OF THE RISK.
YOU acknowledge and understand the following:1. Participation includes possible exposure to and illness from infectious diseases including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist to yourself and all your children and next of kin that participate.2. You knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of the Released Parties; and3. You hereby knowingly assume responsibility of the risk of injury, harm and loss for yourself and your children and next of kin, associated with the Activity, including any injury, harm and loss caused by the negligence, fault or conduct of any kind on the part of the Released Parties.

MEDICAL ACKNOWLEDGMENT AND RELEASE.
you acknowledge the health risks associated with the Activity, including but not limited to transient dizziness, lightheaded, fainting, nausea, muscle cramping, musculoskeletal injury, joint pains, sprains and strains, heart attack, stroke, or sudden death. You agree that if You experience any of these or any other symptoms during the Activity, you will discontinue my participation immediately and seek appropriate medical attention. YOU DO HEREBY RELEASE AND FOREVER DISCHARGE THE RELEASED PARTIES FROM ANY CLAIM WHATSOEVER WHICH ARISES OR MAY HEREAFTER ARISE ON ACCOUNT OF ANY FIRST AID, TREATMENT, OR SERVICE RENDERED IN CONNECTION WITH YOUR PARTICIPATION IN THE ACTIVITY.

As a participant, volunteer, or attendee, You recognize that your participation, involvement and/or attendance at any Summer Camp by Bellevue VBS fundraising event or activity (“Activity”) is voluntary and may result in personal injury (including death) and/or property damage. By attending, observing or participating in the Activity, You acknowledge and assume all risks and dangers associated with your participation and/or attendance at the Activity, and You agree that: (a) the Summer Camp by Bellevue VBS (b) the property or site owner of the Activity, and (c) all past, present and future affiliates, successors, assigns, employees, volunteers, vendors, partners, directors, and officers, of such entities (subsections (a) through (c), collectively, the "Released Parties"), will not be responsible for any personal injury (including death), property damage, or other loss suffered as a result of your participation in, attendance at, and/or observation of the Activity, regardless if any such injuries or losses are caused by the negligence of any of the Released Parties (collectively, the "Released Claims"). BY ATTENDING AND/OR PARTICIPATING IN THE ACTIVITY, YOU ARE DEEMED TO HAVE GIVEN A FULL RELEASE OF LIABILITY TO THE RELEASED PARTIES TO THE FULLEST EXTENT PERMITTED BY LAW.

BACKGROUND CHECK CONCENT FOR VOLUNTEERS
You hereby consent to Summer Camp by Bellevue VBS and Calvary Chapel Eastside verification of all the information you have provided on my application form. You also agree to execute as a condition of volunteer or a condition of continued volunteering any additional written authorization necessary for the Organization to obtain access to and copies of records pertaining to this information. With regard to the foregoing disclosures, You hereby agree to release any person, organization, or other entity from any and all causes of action that otherwise might arise from supplying the Organization with information it may request pursuant to this release. You understand that any false answers or statements, or misrepresentations by omission, made by You on this application or any related document, will be sufficient for rejection of my application or for my immediate discharge should such falsifications or misrepresentations be discovered after You are a volunteer.

RELEASE OF ALL CLAIMS AND AUTHORIZATION FOR MEDICAL TREATMENT
You, hereby give my child, permission to Summer Camp by Bellevue VBS and Calvary Chapel Eastside, Bellevue, WA. I hereby release and agree to hold harmless, Calvary Chapel Eastside and Summer Camp by Bellevue VBS, together with its agents and employees from all actions, causes of action, damages, claims or demands which I, my heirs, executors, administrators or assigns may have against Calvary Chapel Eastside and Summer Camp by Bellevue VBS for all personal injuries, loss, or damage, known or unknown, which we may incur by participating in the above activity. You, have read this release and understand all its terms. You execute it voluntarily and with knowledge of its significance.

The agreement, being being binding with the the parents and /or legal guardians of the children being registered, have made, constituted and appointed, and by these present do make, constitute and appoint any agent of Calvary Chapel Eastside and Summer Camp by Bellevue VBS, their true and lawful attorney-in-fact for them and in their name, place and stead, and for their use and benefit to admit you to any hospital or clinic and to authorize any medical treatment, including surgery, in the event of emergency illness, as Calvary Chapel Eastside and Summer Camp by Bellevue VBS may deem appropriate. Any hospital, clinic or doctor may rely on a telephonic communication reasonably believed to be from an agent of Calvary Chapel Eastside and Summer Camp by Bellevue VBS.

You further agree to assume full financial responsibility for any and all charges incurred, specifically including ambulance, doctor, hospital or medication. The original of the Agreement shall be irrevocable until physically destroyed. Any party relying on this Agreement is hereby released from any liability by reason of relying on this Agreement, or by this Agreement having been revoked without his/her knowledge. You are authorized to give this Limited Power of Attorney. You further promise to hold harmless Calvary Chapel Eastside and Summer Camp by Bellevue VBS and/or its employees and agents from any and all expense incurred pursuant to this authorization in obtaining medical treatment and/or transfer, including but not limited to: ambulance expense, costs of paramedics, hospital expense, and/or physician charges.

LAST MODIFIED. This Agreement was last modified on April 12, 2010.
